Medication Form
EVERY STUDENT traveling to the state marching contest must fill out this form for the State Marching Contest trip. This medication form is to help the directors keep track of your students' medication needs. Parents must sign the form on the top left-hand side before it is turned in. All provided medication must be in the original labeled bottle (prescription meds should have the student's name on the bottle). If your student has an over-the-counter (OTC) medication or prescribed medication, specify the type of medication they need, the dose, and what time of day they need it in the top right-hand corner. The directors will have a limited selection of OTC medication in the first aid bag for emergency use only.
-
If you want your student to have OTC medication for pain relievers, headaches, etc., please provide that medication in the original bottle.
-
If your student does not use daily medication, turn in the form BLANK and signed (this means in the event of an emergency, directors can use OTC Medication to treat your student.)
Please print out the form or grab one from the band hall to fill out. The medication form (and specified medication) is due on the morning of Oct. 31.
